In de province of Friesland, the N381 had a standard profile for a motorway, the road was relatively narrow around Appelscha and had single-level junctions.
In addition, there was a dense urban development between Wijnjewoude and Oosterwolde along the road. In 2014 and 2015, the province of Friesland reconstructed a large section of the N381 to become a fully-fledged, provincial road with good traffic flow. The entire itinerary was expanded with overpasses, partly shifted to a new route between Wijnjewoude and Oosterwolde, and partly widened to a 2x2 lane road between Drachten and Donkerbroek.
